Version: IPYF-WVR-1 (November 2026) | Program: Inland Peak Baseball and all Inland Peak Youth Foundation team programs
This agreement is signed by the parent or legal guardian ("I") on behalf of themselves and the participant named in the registration ("Participant"), a minor. It covers Participant's tryouts, practices, games, tournaments, travel, training and team events in any program operated by Inland Peak Youth Foundation, a California nonprofit public benefit corporation (the "Foundation").
1. Released Parties
"Released Parties" means the Foundation; its directors, officers, coaches, staff, volunteers and agents; Inland Peak Performance and Wellness Inc., whose facility the teams use; and the owners and operators of fields, facilities and events the teams use.
2. Assumption of Risk
I understand that baseball, softball and athletic training involve inherent risks, including being struck by a ball, bat or another player; collisions; falls; muscle, joint and bone injuries; concussion and head injury; heat illness; and, in rare cases, permanent disability or death. Risks also include travel to and from events and the condition of fields and equipment. I knowingly and voluntarily accept these risks for Participant and myself.
3. Release of Liability
To the fullest extent allowed by law, I release, waive and agree not to sue the Released Parties for any injury, illness, death, loss or damage to Participant or me arising out of Participant's participation, including claims based on the ordinary negligence of the Released Parties. This release does not apply to gross negligence or intentional misconduct.
4. Indemnity
I agree to indemnify and hold harmless the Released Parties from claims brought by or on behalf of Participant that are released under this agreement, including attorney fees, to the extent allowed by law.
5. Medical Authorization
I confirm Participant is physically able to participate and I will tell the coach about any medical condition, allergy or medication that affects participation. If I cannot be reached in an emergency, I authorize the Foundation's coaches and staff to obtain first aid and emergency medical treatment for Participant, and I accept responsibility for the cost of that treatment. Under California law, a Participant suspected of a concussion or head injury is removed from activity for the rest of the day and does not return until cleared in writing by a licensed health care provider.
6. Photos and Video
Unless I opt out on the registration form, I allow the Foundation and Inland Peak to use photos and video of Participant taken in public team settings for team, program and Foundation communications, without payment. Participant's last name is not published without my permission.
7. Conduct and Child Safety
Participant and I agree to follow the team's rules and the Foundation's Child Safety and Abuse Prevention Policy, to treat coaches, players, officials and families with respect, and to report any safety concern to the Foundation's President or a director. The Foundation may remove a Participant or family member for conduct that endangers others.
8. General
This agreement is governed by California law and stays in effect for 12 months from signing, or until I sign a newer version. If any part is found unenforceable, the rest remains in effect. I have read this agreement, understand that I am giving up legal rights, and sign it voluntarily. A typed name submitted with the registration form is my electronic signature.
